Moral of this story is to ensure that you are current with the PU's. I wanted to install PU3 last week (prior to reading notes) and controller declined to. So I had to try to fix issue again yesterday. Still not working. As I was about to hit the email send explaining that they were going to have to call tech support for additional troubleshooting, I decided to look at the PU3 notes. Problem had been resolved with PU2 (although it erroneously mentions Premium only).
