I created a Custom report with just PR_EmployeeHistoryHeader, then used Report Manager to add report to menu - checking the box to add the PII work table.
Opened the report in crystal - no work table had been added.
Launched the report from the custom reports menu, let it update to current version, re-opened the report in crystal - still no PII work table.
Tried creating a report with only PR_Employee to the report and repeated the same steps - still no work table
Tried manually adding the PII work table in crystal, now when running the report get an index error (didn't capture the message text).
Threw hands in air, went to bed - sheesh!

Did even realize a KB existed..... Pretty much as Dan outlines. Question is do you add the PII table to your report first or write the report using the PR.Employee table first, then add the report to the menu (which adds the PII table) and then go back replace the PR.employee fields with the PII fields. I've done it both ways.
I can't say enough about Dan's enhancement. It is such a great tool that I include it with practically all of my PR 2.x upgrades. Any company that does ad hoc PR reporting via Access or Excel HAS to have it.

When you ran the report manager, did respond "Yes" to the question "Do you want to include a PII worktable with this custom report?" Did it automatically link the PR_EmployeePIIWrk table into your report? Did you replace the empty PR_Employee PII fields in your report with the PR_EmployeePIIWrk equivalents?
You won't be able to refresh the report in the full version of Crystal - only when you run it from the Custom Reports menu. I have a tool that I'll give you to test custom reports directly in the full version of Crystal if you want to shoot me an email.
