So many things tried. I won't burden you with all the details, but the most recent actions were:
Deleted all report settings for PR Check printing, cleaned out SY_ReportSetting, SY_ReportOption, SY_Lastformused; IT reinstalled printer and swore up and down duplexing was turned off. Long Long story short, created brand new report setting, replaced the customized .rpt. When testing, it printed duplex until we opened Printer Setup from the Check Printing window and disabled Duplex printing (and yes, this was after I had already changed SY_ReportSetting, per @Kevin Moyes' suggestion days ago).

Oh, an upgrade from v4.5. Interesting. What I have done in the past is clean up the clutter via DFDM in ..\SY_ReportSetting.M4T, ..\SY_ReportOption.M4T and \SY_LastFormUsed.M4T that has built up over the years. Users come and go, workstations come and go and form get created that no longer have purpose. I usually delete that I know are no longer used but occasionally I clean up everything but depending on the level of my analness that day, I have deleted everything but the standard form records. Does it help? Dunno but my OCD was tamed for the day.
If anyone is aware of a more efficient method of resetting to "factory settings", please post.

They were on v4.5 before the upgrade. I made changes in SY_ReportSetting to make them the same as the standard form code, but can't test before tomorrow. I'll report back success.

Oh, I see. On the STANDARD form that field is set to "0". For the customized form, it's set to "2". I wonder where it gets that value.
Not that I know what either value means, but I'll back up the file and set it to "0".

The problem seems related to how Sage stores printer settings, not related to the printer selected (or Windows print object). Query SY_ReportSetting to see what I mean.

Both IT and I have looked through printer settings and haven't identified anything there. One of the printers doesn't have double-sided at all. On that computer/printer, it prints every other check.

Not sure this applies to PR checks, but generally speaking there can be things like tray selection stored in a Sage table... settings accessed from the printing screen.
