Have you tried right clicking on the Sage Icon and selected Run as Administrator? If you uninstall/reinstall the PDF Converter, Launch Sage using Run as Admin to see if that bypasses a few permissions issues.

Review KB IDs 19434 and 30079
Restart the Print Spooler
Try printing to a different printer
In the Crystal Reports, make sure the Perform Groupoing on Server and Use Indexes or Server for speed are checked
Also in Crystal, make sure the No Printer checkbox is checked
Uncheck the box in Printers and Scanners that says "Let Windows Manage my Default Printer
Set the default printer to something other than Microsoft XPS Document Writer
Reboot the server
If Sage Standard, make sure the workstation is installed from mapped drive, and for Advanced/Premium from a UNC Path.
Turn off PEP
If it's a Virtual Server, check out KB IDs 71232, 76519 and 52005
Do they use StarShip? See KB ID 112181
Had a customer recently who was having issues getting kicked out of Sage. Determined her NIC hasn't been updated for a very long time. They installed new drivers and she hasn't had an issue since. Probably good to do on server and workstations.

This is when running from the server. IT insists excluded. Antivirus can be doing other things. Was just told system processes can take 60% and it is what it is and it shoudl go away in a few minutes. Also had at least one time out do to another producing a register to paperless.

Have you/they tried running the extended stub report from the Sage Server itself to see how long it takes? My guess is it would go significantly faster. Given AV tasks are using up server resources, it doesn't sound like the Sage program is getting excluded from the AV Active Scans. Make sure you send them the list of AV and Firewall exclusions and apply those exclusions to both Server and Workstation. Also make sure Windows Defender is completely turned off if there's 3rd party AV software present.
Does anything else run really slowly? Seems to me like Sage in general would run very slowly if the above is true. Have you tried uninstalling/reinstalling the PDF Converter? Doesn't sound like that's the issue though if the emails are still being sent out. A normal amount of time to me would be 2-3 seconds per email and maybe upwards of 4-5 depending on the size of the document.
