I think the only suggestion I have is to eliminate the "Other (please specify)" from all of the questions. We're wanting a more direct response and have plenty of other text fields for people to provide "gray area" feedback to some of the other questions. For example, question #1, I would want a simple 1 through 5 metric here so we can aggregate/average the overall satisfaction without having to think of a subjective way to score any of the "Other" responses.
The questions are all otherwise very relevant and presented in a way that will allow people to provide lots of feedback in many different ways.
Lastly - I can't easily tell which questions are required vs. optional. Are all questions required to submit the form? Some? If we can pick/choose, I would have all ratings-based questions (1, 5, 7, 8, 11, and 12) required along with questions 2, 3, and 4.
Even if someone is pressed for time, they can quickly answer A/B/C style questions and type out at least three quick responses in 5 minutes or less.
Normally, I'd hope for better engagement, but I don't know if anyone is going to balk at a second survey on the event (I doubt it, but trying to keep any grumble factor contained).
