As for the survey itself, we've got the following:

I think - as with all surveys - I would add something along the lines of "Additional Details" that allows for further notes separate from the actual tip/trick. With that, we have a brief, 5-question survey that can be filled out in 5 mins or less (unless the consultant gets really verbose in their insights).
Any other suggestions for the survey? Time is somewhat of the essence, so if we can get this to Anne, she won't be waiting on us to get that to her for execution. :)

I can create a survey if that's the method we want to pursue. I use Formstack for this stuff (free for me), and can ensure all responses get funneled where we want them to go (Tina, Alnoor, me, etc.). Results could go to one person, two people, or every one.
It takes all of 5 mins to build the form once we know what fields we want on there.

On a different note, I like Wayne's idea of having many people submit tips and tricks and then have a couple of presenters compile and present them. I think I would need to find the presenters first and then ask people to submit directly to them vs. posting in advance on the community. I second @Brian Kelly's vote... To be more clear, I'll rank them in my order of preference (the "3 votes, 2 votes, 1 vote" concept) as I liked each for different reasons:
- Jeff De Cagna - 3 pts
- Simone Ahuja - 2 pts
- Joanna Shae - 1 pt
I honestly think any of these three would be good to bring to the event, but I do think Jeff and Simone are both timely (navigating disruption) and a good fit (both smart and funny). If anyone else needs some references, I enjoyed each of these videos and the presence that they have:
