@Jim Woodhead you don't need to list the original and amended motion. You just need to record the motion as approved.
Instead of "unanimously consent" I think you just say "passed by unanimous consent". If you're going to note "no abstentions", you need to be consistent.
Also, the written reports should be part of the final minutes.
Otherwise, it's as I recall.
