    I am in the process of revieiwng all of our scope documents (Implementations, upgrades, custom programming, etc.) and was wondering if any of you might be willing to share copies of your forms? We are currently using an Ed Kless Hybrid scope document. I'm in need of a good milestone tracking document and a doc that can be used at the close of a project to review each milestone, it's completion and have the client sign off on it.     Can you define a little more about what is meant by Change Management Services? Also we've found that the attached upgrade agreement has worked well for most of our engagements. I'd probably only go with more formal sign offs on agreements over a certain dollar amount. We've used this on upgrade up to $20k. Most of the time we find customers aren't all that interested in participating in managing their upgrade so we build in that we'll have additional time to perform some tasks for them. I started off using a one page upgrade agreement for fixed fee work. Attached is what I developed over time and I think it covers most eventualities. Not perfect but has worked for us.

    Pam, Issues Lists and After Action Reviews should do the trick. We use Issues Lists in collaboration with customers. We use AARs internally but there's no reason why you couldn't go through them with customers.
