​I have the latest "90 Minds Expense Report & Reimbursement Request" form which includes some details/instructions. Is this it, or is there a separate procedure document I have not found? Thanks.

If it looks like the attachment, yes.  It's an Excel Template; if you copy it to your template directory for Excel, you can access from the Personal Template tab of Excel New, Personal.  You can find your Excel Template directory by selecting, within Excel, File, Options, Save and locating the Default personal templates location.   Most of the instructions are included on the form John, please let me know if you have any questions.
