    Thanks @JeffSchwenk for the ROR form (no, I'm not stealing any of your clients!) but I was really looking for the change request form like the one @MoiraGoggin offered. I also received one from @PamScott very similar to hers. You know, the whole fixed price thing - what to do when the project's scope changes... And thanks!

    Would you be able to pdf that @MoiraGoggin . I've been pretty casual in my change requests -- and find that the biggest issue is that with customers that you have MANY requests that you somehow number/track them. This is especially true for larger complex reports where all changes might apply to the same report.

    PS - I invoice everything out of Freshbooks so my change requests are what Freshbooks calls a Quote - it goes out via an email link. Customer clicks to accept. I have found that to be a really good way to ensure more than a verbal ""ok go ahead"".. FWIW

    We bid each Change Request as a project with three options so the client has choices of delivery options when it is presented (as opposed to simply ""let's do it"" or ""sorry, not interested"").

    @WayneSchulz - you asked for the PDF version of the Change Request....we'll here's a .doc version so you can change what you want. This was from Ed Kless's class.

    I showed the picture to my son who is trying to be a developer and he sort of got the joke. I showed it to his friend who just graduated from the University of Washington Foster School of Business and he had never heard of a change order. Simply amazing.

    The picture is from either Ron Baker or Ed Kless. It was in the slide deck from the FOTF symposium in April.
