What I don't understand is that when this happens for me in User Maintenance, it doesn't happen for all records, only some. There is no commonality among the records for which it does occur (or among those for which it doesn't occur).

If it happens after an upgrade such as the case with User Maint and Payment Type Maint, then like @David Overholt said it's usually because there was a new field in the table (in the dictionary and clicking Convert in Company Maint probably should've taken care of it meaning it's probably a small bug).
For Employee Maintenance this probably wouldn't be expected. One reason could be a script that fires off in the PostRead event (meaning when you choose the Employee No) that does a SetValue to a field in PR_Employee for whatever reason. When you click the arrow to move to the next record, since there is a pending save from the SetValue, it would force the prompt "Do you want to save changes?" To check to see if that's the case, go to UDF Maint, find PR Employee Master, then rt-click and choose User Defined Scripts.
