Okay, figured it out. If you have more than two options and you have an option interaction for only two options (e.g. you want an option from category 1 and an option from category 3 for the interaction bill), it ignores that interaction when you run the gross requirement report unless you only select those two options. In most cases, the customer wants the interaction bill item(s) no matter what the other options chosen are.
In my test:
Bill DSKTP-00200 has three options
For the option interaction, I chose an option from category 1 and an option from category 3 - added an item.
Ran the Gross Requirement report for it choosing an option from all three categories - the additional item does not appear.
According to a KB article:
Example B: If you want option A1 in category 1 and option C7 in category 3, type A1,,C7.
The system assumes that you did not want any options for category 2 or categories 4 through 9.
The problem is, my customer has 9 options for each bill. He can't possibly do interactions for ALL of the options in ALL nine categories if he only wants it if two specific options are chosen. Ironically, the interaction item IS included in a Costed Bill of Materials report.

I just created simple BOM with 2 options and an interaction the if Option1 and Option 2 were on the bill to add a new component item with qty of 5 and delete a qty of 1 for a component that was on the main BOM
then I ran the Gross Requirements report for that bill and both of those options and my report did include the extra component with a qty of 5 per bill. and removed the component that I set to a negative 1. it all worked as I would have expected.
I did this in version 2018. Is that not what your client is trying to do unsuccessfully or did I miss the point?
