Anyone have a suggestion for alternative for SO-1121 (Multiple orders combined into one invoice?) Or is it the only game in town for providing that functionality?

If you want the invoices to update the same way they print, I think that's pretty much it. I've got a few people using it - I really like the ability to explode the orders back the way they were if you delete the invoice.

I've heard the code has some issues when it comes to things like credit memos and handling of back orders...

I don't think my clients have ever used it on a credit memo. Seems to me there is something about back orders, but it depends on which way you check the options.
